Healthy Kids Survey
Fletcher Walker Elementary students in grades 4, 5, and 6 are being asked to be apart of the California Healthy Kids Survey, sponosored by the California Department of Education (CDE). This is a very important survey that will help promote better health and well-being among our youth, improve the school learning environment, and combat problems such as drug abuse and violence. Your child does not have to take the survey. Participation is voluntary and requires your permission.
Westwood Jr. Sr. High School students in grades 7 through 12 are being asked to be apart of the survey as well. Your child does not have to take the survey. If you do not want your child to complete the survey, you must notify the school by calling 530-256-2311 or returning the passive consent form.
Consent forms to opt in or opt out of the survey are being mailed home the week of November 21-25. Please fill out the form and return it to the school as soon as possible. We appreciate your committment to WUSD and look forward to sharing the results from this annoymous survey.
